What is the most likely diagnosis?

Answer: Perinephric hematoma after renal transplant

Case Discussion:

MR images showed a subcapsular renal hematoma.

Perinephric fluid collections are commonly seen after renal transplantation.

Early post-transplant period (<4 weeks); hematoma, seroma, urinoma. Late post-transplant period (>4 weeks);lymphocoele, abscess.
